Hospice Patient & Hospice Team

At Marcus Daly Hospice we recognize how difficult it can be for the patient and family when someone is nearing the end of life. Our Hospice Team focuses on making this transition as smooth, peaceful and comfortable as possible. They focus on patient comfort, family education and addressing whatever the needs are at any given time.

Each patient is given a Hospice Interdisciplinary Team (IDT) who after a thorough assessment develops a care plan that meets each patient's individual needs for pain and symptom control, medical and psychosocial support.

The Marcus Daly Hospice care team consists of:
Patient's Physician
Nurse/Case Managers
Hospice Physician (Medical Director)
Social Worker
Therapists, PT & OT
Chaplain
Pharmacist
Volunteers
Hospice Director

As a part of the care team, typically a family member will serve as the primary caregiver and, when appropriate, help make decisions for the terminally ill individual.

Hospice doesn't take the place of the primary caregiver, whether that is the spouse, an adult child or a paid care provider. Hospice staff and volunteers come alongside the patient and the caregiver to assist, educate and enhance the quality of care at the end of life.

Members of the Hospice team make regular visits to assess the patient, provide additional care or other services and educate the caregiver(s) regarding end of life care. A hospice nurse is on-call 24 hours a day, 7 days a week.
