These professionals provide support and assist with transitions from hospital or nursing home so that you’re able to recover and manage your health status in the comfort of your residence. Marcus Daly Home Care staff works under your doctor’s directions to develop a plan to meet your health care needs. These professionals work as a team to carry out a detailed care plan tailored to address your specific health and recovery needs. Nursing Care Nursing Care is one of the main services offered through Marcus Daly Home Care. Registered nurses (RNs) and licensed practical nurses (LPNs) provide skilled services such as injections and intravenous therapy, wound care, education on disease treatment and prevention, and patient assessments. RNs may also provide case management services. They can help you with medication management, making sure you’re taking your prescriptions on time and in the correct manner. They can also assist you with pain and symptom management. Physical Therapy Marcus Daly Home Care Physical Therapists help you overcome or adjust to physical disabilities following an injury, illness or surgery. They evaluate home safety and other parts of mobility and functions, such as strength, balance, endurance, range of motion and pain. The MDHC physical therapists create a plan of care that includes setting functional goals, teaching home safety, caregiver training, mobility training, therapeutic exercises and instruction in a home exercise program. Home Health Occupational Therapists work with you in your home environment to achieve or regain a higher level of independence. Services include a comprehensive assessment of your ability in daily living activities, such as self-feeding, grooming, dressing and bathing, as well as higher levels of activity like cooking and homemaking. Evaluation also includes assessment of your cognition, upper extremity range of motion, strength and coordination. A personalized treatment plan is created with you and your family or caregivers. It may include instruction in self-care activities, cognitive retraining, caregiver training and recommendations for adaptive equipment or home modifications. Medical Social Work Another service Marcus Daly Home Health offers to clients is the expertise of medical social workers. They help patients and their family members identify available community resources as well as with their Advance Directives. Social workers assist people to function to the best of their ability in their environment, and this may include long range planning after discharge from home care such as meals-on-wheels, personal care assistance or transportation. Speech Therapy Marcus Daly Home Care Speech Therapists assess speech and language development and to treat language and speech disorders. These disabilities are usually a result of hearing loss, stroke or brain injury/disease. Speaking, using sign language, writing and using mechanical and computerized communication devices are some of the most common ones. Therapy that helps speech also helps people with swallowing disorders learn to overcome and cope. Speech Therapists retrain patients in breathing, swallowing, and muscle control and teach techniques to patients and caregivers. Lifeline® gives them the security of getting assistance quickly if they need it 24 hours a day-365 days a year. Lifeline® subscribers press a Personal Help Button, worn as a pendant or wristband, to indicate that they’re experiencing an emergency. Within seconds, a certified Lifeline® monitor responds, assesses the situation and summons appropriate help, such as a neighbor, relative or ambulance. Choose Experience Marcus Daly Home Care has been serving the Bitterroot Valley since 1987. One call to set up a meeting with a case manager, ideally in the client's home, is all it takes. They will work with the client's physician to develop an individualized health plan to promote the client's well being and make it possible for them to remain in their own home.
