Skilled Nursing Care
Our Registered and Licensed Practical Nurses collaborate with your physician
to provide medical management of your acute or chronic illness, education
on disease prevention and coordinate your care with other team members.
Our Physical Therapists and Physical Therapy Assistants evaluate your strength,
balance, endurance, range of motion and pain and then create an individualized
plan of care that includes therapeutic exercises, instruction in a home
exercise program, assistive device recommendations and caregiver training.
Our Licensed and Registered Occupational Therapists work with you in your
home environment to help you achieve your highest level of independence
and safety with your activities of daily living such as grooming, dressing,
bathing, cooking, and homemaking. They also provide recommendations for
adaptive equipment and home modifications.
Medical Social Work
MDMH licensed Social Workers assist you and your family members with short
term counseling services, accessing available community resources, processing
important documentation for long term planning such as Advance Directives,
determining appropriate living arrangements, and managing finances.
MDMH licensed Speech-Language Pathologists work with you to help you speak
more clearly and improve the overall success of your communication allowing
for optimal interaction in your daily life. We also assist you in developing
or regaining the chewing and swallowing skills necessary for safe and
enjoyable eating. By combining education and innovative approaches you
will have the tools to successfully participate in socially fulfilling
Home Health Aides
MDMH Certified Nursing Assistants and Home Health Aides assist you with
personal care needs such as bathing, grooming and hair care as well as
with simple, prescribed exercises up to several visits per week. They
are an important part of the team in monitoring changes in your condition
and assisting you in reaching your goal of independence.
